Motor Vehicle Excess Claim Form
For reimbursement of motor vehicle insurance excess. All claims must be submitted through your Club President and/or Secretary.

- Public Liability Certificates — Standard versions are available on the Lions Insurance website. For named insured versions (where a council or organisation requires their name as an additional named insured), email your requirements with full details of the named party. Please allow at least 10 days before your event.
- New policies and endorsements — Email the Lions Service Team your requirements and a quote or cover note will be provided.
- Claims — The Lions Service Team handles all claims and directs them to the appropriate insurer's claims department. Claim forms are available for download on the Lions Insurance website.
- Risk Management and Child Safe — The Lions Insurance, Risk Management & Child Safe Committee remains available to assist clubs. PDG Paul Cairnduff and PDG Donna Hedges are the specialist contacts for these matters.
Preferred contact method: Email is the preferred way to contact the Lions Service Team. Putting your enquiry in writing reduces the chance of misunderstanding, though phone contact is also available on 1300 242 136.
Motor vehicle claims: All motor vehicle claims must be submitted through your Club President and/or Secretary before being lodged with the broker.
